Admissions
At Linden Grove Health Care Center we do all we can to expedite the admissions process for our residents.
Generally, most admissions paperwork will be handled by your physician’s office, by the hospital that makes your referral, or by our center. We also accept self-referred residents and residents whose families complete the admission process on their behalf.
If you (or a family member) are currently a resident in a hospital and you wish to be admitted to Linden Grove Health Care Center, your best course of action is to inform your physician and/or hospital case worker as soon as possible of your preference.
